Parents of Athletes

 

 

As a parent of a Westwood Athlete there are probably a lot of questions concerning the health and well being of your son (s) or daughter (s) during their participation in one of the many athletic programs at Westwood High School.  The Sports Medicine Department exists for the expressed purpose of caring for these student athletes.

Participation Requirements:

Each athlete at Westwood must have a UIL Participation Form on file in order to participate in any athletic program at Westwood, including off-season, and athletic periods. Click on the link below to display and download a copy of the 2006-2007 athletic packet and physical form.  Print the form. Complete the form, making sure all blanks are filled in and we have your Student ID number. Also make sure form is signed in all places by the student athlete and a parent/guardian. The physical must be on the official UIL physical form which is included in this packet (Updated 1-11-2006).

 

The emergency card can not be downloaded and will need to be picked up in the training room.

Seeing Your Family Physician:

Please make sure you help us with the following information if you take your son or daughter to the doctor. Whether a Westwood Athletic Trainer or a parent initiates a doctor’s visit for any injury, illness, or dental visit, written communication from the doctor is required prior to returning to activity. You must come by the training room to get a Physician Referral Form before going to the Doctor. This form will give us written communication that include diagnosis, level of activity, any restrictions, and what treatment and/or rehab is recommended.


Student Trainers

Student Trainers are a vital part of the overall athletic program. Student Trainers will be assigned to all sports and teams.


Training Room Hours:

Normal training room hours are 7:30 AM to 5:30 PM. Times can vary slightly due to games, and will be posted outside the Training Room.


Team Physician

Our Team Physician is David Kerr M.D. Doctor Kerr is at most football games, and is available to the athletic trainers for consultation.
